The collaboration aligns two organizations committed to improving health outcomes through innovation, strong clinical execution and a shared belief that better health starts with strengthening the physician-patient relationship. By partnering with Matrix, agilon will work with Matrix’s nationwide network of experienced Nurse Practitioners and Physician Assistants performing comprehensive in-home health and care assessment to connect members with their primary care providers and ensure they receive timely and necessary care. Through these home-based encounters, Matrix clinicians will help assess medical and social risk factors, facilitate connections to primary care providers and deliver education that promotes preventive and chronic care management.
