For nearly 25 years, Matrix has been caring for Medicare Advantage, Managed Medicaid and Commercial members where they’re most comfortable: in their homes. Acting as a bridge to care, above and beyond a patient’s annual primary care physician visit, we conduct comprehensive health assessments focused on the core pillars of health unique to each individual: physical, emotional, spiritual and social. No matter how long it takes, we make sure that the people we see are in a better place when we leave than when we walked through their door.

Comprehensive Health Assessments
Our clinicians take the time that is required to understand each patient as a whole person, including their physical, emotional, spiritual and social health. In partnership with your health plan, we provide care in the convenience of your home.
Personalized Care
We’ll meet you where you’re most comfortable, and wherever you are in your personal health care journey. We understand that care has many layers, and it’s personal. We’ll do everything possible to care for you on your journey towards better health. This includes coordinating support you need such as a follow up with your primary care physician and other health providers or non-clinical services. Together, we can make informed decisions about your health.

What is a comprehensive health assessment?
At Matrix, we believe in whole-person care — personalized support and undivided attention that may not be available during regular visits with your primary care physician. Our comprehensive health assessment is a unique opportunity for you to spend 45-60 minutes of undivided, personalized attention with a highly-trained and board-certified nurse practitioner.
Physical Health
In addition to physical examinations and a review of personal and family health history, our clinicians work to identify risks, ensure proper medication management, discuss lifestyle improvements and more.
Emotional & Spiritual Health
Our clinicians hear our patients’ unique stories and assess their individual challenges first-hand. They are the eyes and ears of care — often seeing things others don’t, including the real person in front of them striving to feel better, to do more, to live a healthier, fuller life.
Social Health
Whether we’re connecting people to pharmacies or food banks, transportation or shelters, we understand that care has many layers. We do everything possible to support, educate and care for individuals on their journey towards better health.
What to expect
Your Comprehensive Health Assessment Visit.
Before Your Visit
To prepare for your visit with your Matrix nurse practitioner, please:
- Gather your prescription bottles and lab results and write down any questions you may have.
- Have your health insurance card available.
- Have the names and phone numbers of your doctors handy.
During Your Visit
A Matrix nurse practitioner will meet with you for 45-60 minutes in the comfort and convenience of your home. Our clinicians follow safety precautions recommended by the CDC, including the use of personal protective equipment like masks and gloves, when needed.
During your visit, you will receive:
- A comprehensive health and wellness assessment
- A full review of your medical history and medications
- Personalized recommendations for additional care, screenings and support resources if needed
- Answers to your health questions and guidance about testing, safety practices and life choices
Your Matrix health clinician may collect blood or urine samples for cholesterol, blood sugar A1C diabetes screening and kidney function testing and send them to the lab. The nurse practitioner may also provide a colon cancer screening kit with a prepaid envelope for you to mail to the lab for analysis.
After Your Visit
Within a month after the visit, you, your primary care physician, and your health plan will receive your Personal Health Summary that shows all the assessment results in an easy-to-read format. Together, you can make informed decisions about your health.
