As the COVID-19 pandemic continues to rage, recognizing selfless contributions of nurses is more important than ever

The American Nursing Association recently extended the traditional National Nurses Week to a month of recognition in May to provide the opportunity to continue to elevate the nursing profession. While Nurses Week is always a cause for celebration of healthcare’s frontline caregivers, this year it has taken on special meaning during the extended month long recognition. It is taking place while the U.S. is deep in the throes of battling the COVID-19 novel coronavirus, which is having a devastating health and economic toll on the country.

“Nurses hold a wonderful contradiction: to be able to walk alongside another person during their moments of vulnerability requires a special type of selflessness and strength,” said Keith Henthorne, Chairman and CEO of Matrix. “Our nurses demonstrate this selflessness and strength daily, but especially during this COVID-19 crisis. The bravery, commitment and willingness to be on the front line of care is to be admired and celebrated.”

In normal times, Matrix’s extensive network of nurses visit patients in their homes or neighborhoods to deliver a breadth of high-quality clinical services to many who may be otherwise disconnected from the healthcare system. Matrix nurses also expend a great deal of their effort on patient activation to ensure health plan members become and remain engaged in their own care.

Since social distancing requirements have caused Matrix to reduce the number of face-to-face contacts its nurses are making to help flatten the curve on the spread of COVID-19, they are expanding their use of telehealth to ensure member needs are met without the risks inherent in direct contact.

Another way Matrix is addressing the pandemic and helping re-open the economy is by deploying its fleet of mobile clinics to help protect employees and create additional healthcare access to those working at essential distribution centers and food manufacturing facilities like Tyson Foods. Once again, Matrix’s nurses are on the front lines to provide an extra benefit that can bring peace-of-mind to employees as they return to work.

SCOTTSDALE, Ariz. (March 18, 2020) – While the COVID-19 pandemic is affecting the entire U.S. population at some level, people at highest risk are older Americans and those with multiple chronic conditions (source: Centers for Disease Control and Prevention). Knowing that these are also the most difficult health plan members to reach, Matrix Medical Network (Matrix) has implemented a targeted COVID-19 program that uses its proven clinical excellence to balance the social isolation that vulnerable seniors may be feeling during the pandemic. Now more than ever, Matrix nurse practitioners (NPs) are helping this population become informed about safe practices and ensure they become active in protecting their health during these difficult times.

“Much of the information coming from government and healthcare sources is being delivered and shared online,” said Laura Jonsson, senior vice president of clinical services at Matrix. “Yet older Americans, currently considered to be the highest-risk population, often get their information from television, radio and newspapers so they may be missing vital steps to protect themselves. By going into their homes and sitting with them face-to-face, our NPs not only deliver important information and ensure members understand what to do to avoid acquiring the virus, they can also look around the home to make sure members have adequate food and medical supplies, uncover any social determinants of health issues that might put them at greater risk and generally ensure they’re prepared to follow the CDC’s guidelines.”

The key to success is Matrix’s 3,000+ licensed NPs who are dedicated to bringing care and compassion to the individuals they see every day despite working now in very uncertain times. The NPs visit with members in the safety and comfort of their own homes while building a trusted relationship with them. These relationships will enable them to bring vital accurate information to many in the high-risk group. It also avoids the need to seek this information by visiting a potential high-contagion area such as their primary care physician’s office. Matrix is committed to screening practices that help ensure the safety of the members they see every day as well as their own providers.

Although anyone is susceptible to acquiring COVID-19, early data showed higher death rates for people in their 60s, 70s and 80s (source: Bloomberg). According to the CDC, seniors are most susceptible because 85% of adults over the age of 65 have at least one chronic condition such as arthritis, cardiovascular disease, chronic obstructive pulmonary disease and diabetes, which exacerbates the effects of COVID-19 by creating medical complications. Seniors who are not receiving care in a skilled nursing facility (SNF), assisted living community or other group situation are often socially isolated as well, which means if they have been exposed to COVID-19 they may not have quick access to care.

“The Internet is filled with all types of misinformation, and even if members didn’t see it directly themselves, it may have been passed along to them through email or a well-meaning friend or family member,” Jonsson said. “We try to make sure that by the time the visit is over members understand what to do as well as what not to do so they can remain safe in their homes until the danger has passed.”

Another target for Matrix’s COVID-19 program are dual-eligibles – members whose healthcare expenses are covered under both Medicare and Medicaid.

“Many in the dual-eligible population don’t have Wi-Fi or mobile phones, so their access to up-to-date information is very limited,” Jonsson said. “With so much changing day-to-day we try to ensure that they understand what is happening now so they don’t panic or accidentally put themselves in harmful situations.”

The COIVID-19 situation continues to be very fluid. As such Matrix continues to seek ways to bring value to the members and communities they serve, while coordinating with its health plan and health system clients, as well as local, state and federal public health authorities to ensure the safety of its corporate staff, providers and members across the country.

A Healthy Heart for the Rest of Your Life

Michelle, a Matrix Nurse Practitioner, recently shared her personal experience with a heart health story and the ways to promote a healthy heart:

My grandmother of 98 years asked that I accompany her to a results visit with her Primary Care Physician (PCP) since I was “almost a doctor” and could help her understand anything if needed. For the record, I am a Matrix Nurse Practitioner and I gladly consented.

We were having a fun conversation when the PCP entered and warmly greeted us as if we were family. He reviewed the results of her lab-work, and then discussed decreasing the dosage on a few medications as she was doing so well.

He discussed continuing her current diet and encouraged her daily walk to the mailbox and back…about 1/4 mile round trip.

He asked if she had questions and spent a few minutes asking about family. He shook her hand and said, “You are such a wonderful person, and have the softest heart I know.”

When we got into the car, she looked angry which is unusual for my grandmother. I asked her what was wrong and she said, “I don’t have a soft heart! My heart is strong as an ox. I don’t think he is a nice doctor to wait until the end of the visit to tell me that!”

I looked at her and said, “He meant that you have a soft spot in your heart for your family, friends and life. He said you were a wonderful person and he meant it.”

She smiled and exclaimed, “He is the nicest doctor I know!”

Whether in the home, mobile clinic or facility, Matrix providers get an opportunity to promote a healthy heart. This opportunity may be presented, or stated in as many different ways as there are providers, but let me leave you with the following:

Help yourself to as much education as you can get. Know what your numbers are, and should be, for your BP, Weight, A1C, and Cholesterol for starters.

Eat a balanced meal that includes a protein, vegetable, starch and carbohydrate. Moderation is key. Be sure to drink plenty of water throughout the day!

Activity should be a part of your daily routine. Start with what you can do, and then do what you can do every day.

Rest and Relax are the hardest to do but most important to plan into your life.

Take time for yourself, family and friends. Share your “soft heart.”

Our “heart” longed to reach more members, so Matrix decided to take our “heart health” mission where they live …in the form of Mobile Health Clinics.

One of the unique features of the Mobile Health Clinic to improve quality and affordability of health care is cardiovascular testing. Our Mobile Health Clinics offer a comprehensive health assessment conducted by a board certified provider, who is able to order an Electrocardiogram (ECG) and/or an Echocardiogram (ECHO) for members who have cardiac risk factors. The tests are performed on board the mobile health clinic by a cardiovascular technician in a comfortable setting.

The ECG looks at the electrical activity of the heart and helps the provider determine the rate and regularity of the heartbeat.

The ECHO looks at the pumping activity of the heart, the valves and chambers, using an ultrasound machine. An Interpreting Physician will evaluate the results and a report is sent to the member’s primary care physician (PCP) to support the goals of quality patient care. The PCP assumes responsibility for treating and referring the member to a specialist if indicated.

These two tests are so important to the heart health of the member as they provide a good measurement of how healthy the heart is, or can be, with the proper intervention and treatment that starts with the Matrix team and continues with the member’s PCP or Specialist.

February is American Heart Month

February is American Heart Month

During the month of February, matters of the heart are often top of mind. From chart-topping songs telling stories of heartbreak to holidays dedicated to the celebration of love, we all seem to be focused on the heart, in a metaphorical sense anyway.

But, behind those multi-colored candy hearts and arrow-wielding cherubs there lies a more serious topic. February marks the beginning of American Heart Month, a federally-designated event that has sought to raise awareness for heart disease and stroke since it was first proclaimed by President Lyndon B. Johnson in 1964.

It’s the goal of organizations like the Centers for Disease Control and the American Heart Association to increase awareness of heart health and lower the risks for heart disease. According to the Centers for Disease Control, heart disease is the leading cause of death for men, women, and people of most racial and ethnic groups in the United States. Both organizations emphasize the importance of regular exams and testing from a healthcare provider.

Within our Matrix Medical Network (Matrix) Clinical Education team, and the Matrix providers in the field, the heart is an organ or pump that sits behind the sternum and sends oxygen rich blood throughout the body with every beat that is electrically initiated. The heart beats at an average rate of 80 times per minute, which is about 42 million times a year.

Thankfully, most of us do not have to think about how or why the heart beats, but ALL of US should be concerned about keeping that heartbeat going strong and healthy for the whole of our lives. Awareness of traditional risk factors, education about heart health and lifestyle modifications can help to mitigate the risk for heart conditions, such as coronary artery disease.

Coronary Artery Disease (CAD) applies to the arteries that supply blood flow to the heart muscle. The diagnosis of CAD indicates that the individual has atherosclerosis or plaque in the coronary arteries. Atherosclerosis builds up and damages the coronary arteries causing platelet aggregation, resulting in a blockage. Atherosclerosis minimizes the blood flow and can lead to ischemia or a myocardial infarction.[1]

Assessing heart health is part of the Matrix Comprehensive Health Assessment (CHA). During the member visit, the Matrix provider spends a portion of the visit asking about issues of the heart…the organ and the vessels in a physical sense that is.

Matrix providers ask questions to assess the member’s risk for heart related disorders, perform a physical exam, and conduct testing. Members are asked if they have a family history of Coronary Artery Disease and when their last Cholesterol screen was done. Providers assess for signs or symptoms that may indicate a heart problem, such as chest pain, shortness of breath, irregular heartbeats, previous surgeries, and history of hospital admissions. Providers on the Matrix Mobile Health Clinic are able to further evaluate the heart using EKG and Echocardiogram testing. The thorough assessment of the whole body system enables the provider to learn more about the overall health of the member’s heart, and to make recommendations regarding following up with their primary care physician or specialist based on these findings.

SCOTTSDALE, Arizona, January 21, 2020 – Matrix Medical Network, a leading provider of in-home and mobile health clinic assessments and care management, and Excellus BlueCross BlueShield, which delivers health care coverage across upstate New York, have partnered to provide a state-of-the-art mobile health clinic (MHC) to ensure that members have even greater choice and convenience in accessing important care and preventive services.

“Many of our screenings can detect disease, diagnose conditions before they become advanced, and help people live longer and healthier lives,” said Dr. Daniel J. Castillo, Matrix chief medical officer. “We are excited to have one of our mobile health clinics on the road in upstate New York and we are very proud of our ability to improve access of care to Excellus BCBS members and help improve their lives.”

Eligible Excellus BCBS members can receive preventive diabetes screenings such as a dilated retinal exam, as well as colorectal screening and mammography. Eligible Medicare members can also receive bone density screening. Appointments will be scheduled only for members with gaps in care based on nationally recognized preventive services guidelines.

To assist Medicare and Medicaid members who haven’t had their preventive screenings, Excellus BCBS is inviting those members by mail to make an appointment and visit the mobile health clinic, which will be at prominent locations in the neighborhoods where these members live. During December, the mobile health clinic staff saw members in New York’s Monroe, Onondaga, Oneida and Broome counties. The program continues into 2020.

“Closing the gaps in care related to preventive screenings is an ongoing challenge for some members and that makes it an ongoing challenge for their doctors and their health insurer too,” said Dr. Brian Steele, Excellus BCBS chief medical director for Safety Net products.

“The goal is to work in partnership with the member’s physician to help the member receive the preventive services they need,” said Dr. Steele. “We want all of our members to stay healthy.”

“The largest part of my job is focused on being a clinical resource and providing education about the electronic portal for our providers, staff and clients. I am always looking for ways to enhance the tools for data collected during the assessment. 

I was once told by a wife of a member that I saw that I saved their marriage. Her husband would not take his medication or follow his diet.  His wife was frustrated that he would not listen to advice given about his diabetes. I educated him about how diabetes affects all the different body systems and why the treatment was necessary. He stated that everyone told him what to do, but never why. His wife called me a month later and told me that he was doing everything he was supposed to and that she was saying with him.

For new NPs, the advice I would give is don’t be afraid to ask questions— no one expects you to know everything. The learning never ends!”

An Amazing Impact

“I believe that most of the members I have seen really appreciate the hour we give them to discuss their health status and feel important when they get the opportunity to ask questions and get a better understanding of their well-being.

The most significant impact on my members was when I was a PA Travel NP for five years. I worked in over 40 counties in PA. I worked in the mountains, the farm lands of Lancaster, Centre County of Happy Valley and coal fracking towns of Schuylkill county and many more! The rural members were even more appreciative when I did their home visits and educated them about health issues, medication treatment and how to improve or maintain a healthy status. The country members were very respectful and appreciated the NP Visit. The impact I personally felt was amazing.”

“I remember the Matrix interview and starting the job so well! I love the freedom and the wonderful members I talked to over the years. I always enjoy meeting members and the facility staff members who I came to know as I visited the skilled and long-term care facilities. Many members were from my hometown and were seen over the years repeatedly. It has been an honor to represent our company as we grew and developed over time. I feel that I took a chance on a new way of doing health care which in reality was getting the care back into the homes as it was done years before us. Confidence in Yourself & Your Skills

“I can’t believe it’s been that long! Back at the start we did all the assessments on paper and sent them via FedEx, and the assessments were about 30 pages long. Now we are one of the largest employers of NPs in the country!

As a Matrix NP I am able to connect the member to their health plan and their PCP all in one visit. Sometimes members do not have access to the services they need and we are able to connect those services to them. I have organized member’s medications into pill boxes for the month, connected the member with their doctors and spoken to family members when I am concerned for their loved one’s well-being.

One piece of advice I would pass on to the next generation of NPs is to have confidence in yourself and your skills. You know more than you think you do!”

A Leap of Faith

“My journey started with a ‘ leap of faith’ in Matrix when they just started doing home health assessments in Pittsburgh. In the ‘old days’ we called our members the night before, assessments were done on paper and sent via FedEx. Fast forward ten years and we have come a long way! Ten years…that’s a lot of miles and many members seen— and I still love doing it!  

To me, each visit is like opening up a new novel…we walk into a home and the story begins. We have a unique opportunity to make a differ-ence in a member’s life every time we walk through a new door— to me that’s what it is all about!”

Quality Time with Members

“I feel that my work as a Matrix NP has placed me in a unique position in today’s health care environment by allowing me to spend quality time with members. By taking the time to listen to each member’s story I’m able to meet the member where they are in their journey to a healthy lifestyle. I hope that my day to day interactions with the members leave them feeling listened to and cared for.

As much as I feel like I’m there to educate our members about their health-they have taught me so much in return. There’s a unique perspective on life that you obtain from listening to our member population, and I’ll take all the advice I can get.

For newly graduated NPs, I would say being able to really listen is an important skill to develop. Sometimes taking a step back can really put everything into focus. And to have fun with the members— they really are a treat.”

Making a Real Connection

“In the ten years here, I have seen the company evolve from my very first team meeting where there were only 12 NPs in the Western Pennsylvania area. Matrix has undergone a complete transformation since when I first started.

I approach each visit with the goal of finding a way in which the member will both remember my visit as well as find value in it. Sometimes the difference is made clinically, but other times it is by just spending a little extra time with a member and taking a genuine interest in the member’s life or passions and making a real connection with them. I have met so many interesting people through the years and remember them fondly when I drive past their home, sometimes years later.

One of my favorite stories was of a member that I saw more than once. During his first visit, there were significant issues, which I addressed. A few weeks later, I ran into him in my community and he stopped me and thanked me profusely, stating that I probably saved his life. It was about two years later that I was scheduled to see him again for his annual assessment. When he opened the door, I didn’t even recognize him. He had lost a significant amount of weight and had a huge smile on his face. As we went through the assessment he talked about the first visit with Matrix and how it had changed his life. These are the types of differences all of us make working here at Matrix.

To the new grads, my advice would be to have confidence in yourself and the education you received. One of my favorite quotes is from Henry Ford who said, “whether you think you can, or you think you can’t— you’re right.”

No Act of Kindness is too Small

“I would have to say that the vast majority of visits I have completed for Matrix have been fulfilling on one level or another. I am always happy when I can take the burden off the facility nurses. Or brighten a members’ day by taking the time to listen to them or make them smile.  The small things sometimes make all the difference to both the members and facility staff.

I also enjoy the family interaction when I have the time to explain how a discharge to home will happen or to head them in the right direction to get their questions answered. When I first began my nursing career, I wanted to be the person who saved lives during a hectic crisis. After 30 years I realize that a kind word or a touch can be just as life saving for someone who is ill.

My advice to new nurse practitioners is to enjoy the small things and remember that no act of kindness is too small to make a difference.”
